Why InsightXtract

Three things that move P&C operations

Most extraction tools stop at OCR. InsightXtract runs an agentic pipeline — extract, validate, decide — with auditability baked into every step.

Understand

Document understanding, not just OCR

Layout-aware extraction across structured forms, semi-structured loss runs, and free-text emails — with per-field confidence scoring.

  • ACORD, SOV, loss run, dec page
  • Hand-written annotations, stamps, signatures
  • Multi-page, multi-section, multi-table
  • Per-field confidence + provenance
Validate

Agentic pipelines, not single-step

Multi-step pipelines: classify the document, extract the schema, run business rules, cross-check against systems of record, escalate if needed.

  • Configurable validation rule library
  • Cross-document & cross-system checks
  • Branch on confidence or business rule
  • Visual pipeline builder
Decide

Straight-through or human-in-the-loop

Configure per LOB, per document type, per confidence threshold. Fully automated where the data is clean; underwriter review where it isn’t.

  • Per-LOB straight-through routing
  • Reviewer queue with full context
  • Reviewer feedback flows back to the model
  • Auditable trail of every decision & override
Platform

The platform behind every pipeline

Governance, human oversight, and continuous improvement built in — so every extraction stays reviewable, controlled, and gets better over time.

InsightXtract high-level architecture — inputs and connectors feed the agentic extraction engine (classify, extract, consolidate, validate), grounded by knowledge and config, governed by RBAC/versioning/audit, improved by human oversight and self-improvement, and delivered via API and workflows to your underwriting workstation, PAS, claims system, and data warehouse
Ask

Chat with a submission or claim

Point at a folder in SharePoint, a mailbox, or your own bucket and ask it anything — with the verbatim words behind every answer.

  • Click a citation and the document opens at the page, words boxed
  • Tables and charts when a comparison is the point — every figure cited
  • Says “the folder does not state this” rather than guessing
  • Runs with the asking user’s permissions; available as a REST API
How it works
Review

Human-in-the-loop review

Reviewers correct extracted fields and fix document classifications with a reason, then approve, reject, or request changes on a run.

  • Correct fields and reclassify documents inline
  • Approve, reject, or request changes per run
  • Every edit and decision captured with who and when
Self-improving

Agents that learn from corrections

Accumulated corrections are analyzed into suggested prompt, rule, and class-description changes for a reviewer to approve.

  • Corrections become suggested prompt and rule changes
  • A reviewer approves before a new agent version ships
  • Validated against the feedback set and golden baselines before promotion
Governance

Roles & separation of duties

Role-based access — Admin, Builder, Operator, Reviewer, and Viewer — so each role sees only what it needs.

  • Five built-in roles scoped to the task at hand
  • Whoever reviews data can’t silently change agent config
  • Least-privilege access across projects
API

Programmatic access & API keys

Invoke agents from other systems through a versioned Extraction API with scoped, version-pinnable API keys.

  • Versioned Extraction API for system-to-system calls
  • Scoped keys you can pin to an agent version
  • Every call tracked for audit and usage
Workflows

Pipelines & delivery

Visual pipelines chain extraction with transform, conditional, webhook, database, and S3 steps.

  • Chain transform, conditional, webhook, database, and S3 steps
  • Trigger by API, webhook, or schedule
  • Post results straight to your systems
Versioned

Versioning & audit trail

Agent and output-contract versions with per-field provenance and a full audit trail across the pipeline.

  • Versioned agents and output contracts
  • Per-field provenance with bounding boxes
  • Full audit trail of extractions, corrections, and approvals

P&C use cases

Four pipelines we ship out of the box

Each pipeline is a starting point — carrier-specific rules, LOBs, and routing get configured in the first sprint.

Most popular

Submission intake

From inbound broker email to triaged submission packet in your PAS — with all attachments parsed, validated, and routed.

  • Email + attachment ingestion (Outlook, IMAP, Salesforce)
  • ACORD forms, SOVs, loss runs, supplemental apps
  • Appetite check + clearance + risk-tier routing
  • PAS integration with full submission packet
Claims

Claims FNOL pipelines

First Notice of Loss from any channel — phone transcript, broker email, FNOL form, partner API — into a structured, validated claim record.

  • Multi-channel FNOL intake
  • Loss type classification & severity scoring
  • Coverage check against policy data
  • Claim handler routing & auto-acknowledgment
Renewals

Renewal data refresh

Pull updated SOVs, loss runs, and exposure data from brokers and partners — with diff against last year for the underwriter.

  • Year-over-year SOV diff
  • Loss-run delta analysis
  • Exposure-change flagging
  • Renewal-pack prep for UW
Reinsurance

Bordereaux processing

MGA bordereaux ingestion at scale — premium, claim, and aggregate bordereaux normalized to your treaty schema.

  • Premium / claim / aggregate bordereaux
  • Per-MGA schema mapping
  • Treaty-aware validation
  • Variance flagging vs prior periods
Lines of business

Comprehensive coverage across Specialty P&C lines

Purpose-built extraction agents per line — each capturing the submission’s full detail: exposures, schedules, loss history, and coverage terms. New lines are configuration, not a rebuild.

Don’t see your line? New agents are authored from field definitions, glossaries, and reference data — typically live in days.

Document types

Built for the documents P&C actually receives

Not a generic OCR plus regex. Pre-trained on real P&C documents, with schemas your team can extend.

ACORD forms

All major ACORD templates with field-level schema mapping — ACORD 125, 126, 130, 131, 137, 140, and more.

Loss runs

Any carrier’s loss-run format — tabular, PDF, scanned. Normalized to a unified loss schema.

SOVs & Excel attachments

Statements of Value, exposure schedules, premium bordereaux — messy Excel turned into clean per-location records.

Broker emails

Free-text email bodies parsed for intent, named coverages, and embedded data. Attachments routed automatically.

FNOL forms & reports

FNOL packets, police reports, medical records, repair estimates — structured into claim records.

Custom forms

Your own underwriting questionnaires, supplemental apps, MGA-specific forms — trained on a few hundred samples.

Deployment

FAQ

Common questions from P&C teams

What carriers, MGAs, brokers, and TPAs ask us first. More in our discovery call.

Which ACORD forms are supported?

Out of the box: the major P&C ACORDs — 125, 126, 130, 131, 137, 140, plus dec pages and certificates. Custom variants and supplements onboarded in days from a few sample documents.

Does it integrate with our PAS?

Yes — pre-built connectors for Guidewire, Duck Creek, Sapiens, Insurity, EIS, plus clearance systems and broker portals. Custom integrations via the connector framework if you run something internal.

Straight-through processing or human review?

Both, configurable per LOB and per document type. You set confidence thresholds and business-rule trip-wires; everything above threshold goes straight through, anything below routes to a reviewer queue with full document context.

What does the audit trail capture?

Every extracted field with confidence, every validation rule that fired (pass or fail), every reviewer action (confirm, correct, escalate), every PAS write — with timestamps and user attribution. Exportable for internal audit and regulator review.

How long to onboard a new document type?

For variants of templates we already support (e.g., a carrier-specific loss-run format), usually 1–2 weeks from sample documents. For entirely new document types, 3–6 weeks including schema design and validation rules.

Where does PII / claim data live?

Self-hosted in your environment — cloud or data center. Documents, extractions, and review queues all stay inside your perimeter. HIPAA / GDPR / SOC 2-compatible deployments.
